Dental insurance, the second most desired employee benefit after medical coverage, comes in many forms.
Dental HMO
Provides in-network dental coverage only. Typically, there are no charges for many routine services and nominal co-pays for others. Usually, there are no deductibles, no claim forms, and no maximums. Orthodontia is available with some plans. While the freedom to choose a dentist is restricted, out-of-pocket expenses are often lower than with other plans.

POS/PPO Dental Plans
These plans allow patients to use either a participating dentist (in-network) or a non-participating dentist (out-of-network). In-network services usually require a co-pay. Out-of-network services are subject to a deductible, coinsurance and claim forms.

Indemnity Plans
Indemnity plans provide freedom to visit any dentist. Usually there is a yearly deductible and reimbursement for covered expenses. A typical plan might cover 100% of preventative services like cleanings, 80% of basic services like fillings, and 50% of major work like crowns and bridges. There is an annual maximum somewhere between $1000 and $3000 and orthodontia may be covered. A minimum participation percentage of a company's employees is required (usually 50-75%). A census of employees' birth dates, gender, dependent status and home zip code is needed to obtain rates.
